Community Health Worker

The Community Health Worker course is geared towards participants who want to work in either a community-based or inpatient setting and contribute to improve health outcomes to the community where they reside. Topics include the role of the community health worker, ethics and professional skills, teamwork and communication, documentation and assessment procedures, chronic disease overview, accessibility to health care services, and care coordination and system navigation. This accredited training program satisfies the educational requirement needed to seek PA CHW certification.

 

Funding

Funding may be available to eligible participants through PA Careerlink Monroe County.
